Coaching Registration

After your one free phone consultation, I will email you a link to schedule and pay for your coaching package. I will also send you intake forms to be filled out prior to your first session.

First Visit: Please book your appointment online and email your intake forms prior to your first session.

Payment: Full payment is expected prior to the scheduled appointment. Visa, MasterCard, Discover and American Express, and personal checks are accepted.

Refund and Cancellation Policy

Coaching packages are non-refundable. Appointments that are canceled 24 hours or more prior to the appointment time may be rescheduled for a future date at no additional cost. Appointments cancelled less than 24 hours will be subject to a $10 transfer fee when rescheduled for a future date. No-shows for a session results in the loss of that session.

Confidentiality Policy

The coaching relationship, as well as all information (documented or verbal) that the Client shares with the Coach as part of this relationship, is bound by the principles of confidentiality set forth in the International Coaching Federation (ICF) Code of Ethics. The Coach agrees NOT to disclose any information pertaining to the Client without the Client’s written consent. The Coach will not disclose the Client’s name as a reference without the Client’s consent.

Confidential Information does not include information that: (a) was in the Coach’s possession prior to its being furnished by the Client; (b) is generally known to the public or in the Client’s industry; (c) is obtained by the Coach from a third party, without breach of any obligation to the Client; (d) is independently developed by the Coach without use of or reference to the Client’s confidential information; or (e) the Coach is required by statute, lawfully issued subpoena, or by court order to disclose; (f) is disclosed to the Coach and as a result of such disclosure the Coach reasonably believes there to be an imminent or likely risk of danger or harm to the Client or others; and (g) involves illegal activity. The Client also acknowledges his or her continuing obligation to raise any confidentiality questions or concerns with the Coach in a timely manner.

In compliance with the CAN-SPAM Act, all e-mails sent from our organization will clearly state who the e-mail is from and provide clear information on how to contact the sender. In addition, all e-mail messages will also contain concise information on how to remove yourself from our mailing list so that you receive no further e-mail communication from us.

Our emails provide users the opportunity to opt-out of receiving communications from us by reading the unsubscribe instructions located at the bottom of any e-mail they receive from us at anytime.

Users who no longer wish to receive our newsletter or promotional materials may opt-out of receiving these communications by clicking on the unsubscribe link in the e-mail. After unsubscribing we will discontinue sending the particular messages as soon as technically feasible.

Surveys & Contests
From time-to-time our site requests information from users via surveys, quizzes or contests. Participation in these surveys, quizzes or contests is completely voluntary and the user therefore has a choice whether or not to disclose this information. Information requested may include contact information (such as name and shipping address), and demographic information (such as zip code, age level). Contact information will be used to notify the winners and award prizes. Survey and quiz information will be used for purposes of monitoring or improving the use and satisfaction of this site.

A Special Note About Children
Children are not eligible to use our services unsupervised and we ask that children (under the age of 13) do not submit any personal information to us. If you are a minor, you can use this service only in conjunction with permission and guidance from your parents or guardians.

The safety and privacy of children online is important to us and we take great care to comply with the Children’s Online Privacy Protection Act of 1998 (COPPA). JenWhitaker.com does not allow this website to be accessed by children under the age of 13 without the supervision of a parent or guardian.
